Tap the "Triage" button on the landing page (Home).

Tap the "CREATE PATIENT PROFILE" button.

The "Profile" page will appear and you will be asked to enter patient information

Enter the Full Name of the patient in "Name". Enter Khmer Name if applicable.

Swipe right every time you fill up a particular information about the patient.

Enter the "Age" or "Date of Birth" when prompted.

Don't click "Submit" until you have completed all the info on the patient you need.

Don't forget to enter Tag Number of the patient when prompted!
